As of July 5th, 2013, the new AmeriChoice of New Jersey search options are as follows:

1.       Required(Subscriber’s Last Name, Subscriber’s First Name, Subscriber’s Date of Birth, Subscriber ID)

2.       Required(Subscriber’s Last Name, Subscriber’s Date of Birth, Subscriber ID)

3.       Required(Subscriber ID, Dependent’s Last Name, Dependent’s First Name, Dependent’s DOB)
